Instructions for TULIP coverage request

to be completed by Live Nation Employees Only

FOR STANDARD VENDORS/RENTERS (non-music event rentals):
  1. Live Nation contact will complete the TULIP request form (Next Step Online)
  2. Live Nation contact will collect premium due (based on calculation from request form) and deposit locally to Account # 21033.
  3. No return receipt or evidence of coverage will be sent to the Live Nation contact unless at the request of the VENDOR/RENTER. Upon submission of the form, TULIP coverage is activated. Only standard craft/food/exhibitor vendors qualify. Amusement, piercing, tattoo, medical services and gaming vendors do not qualify.
